Onboarding Requirements
Upon application approval, you will need to provide the following documents:
- Must obtain a business license from the City of Rohnert Park
- Must submit a signed W-9 Form
- Must complete Live Scan & TB Screen (More information provided upon application acceptance).
- Must provide proof of General Liability Insurance with the following specifications:
- 1,000,000 per occurrence and $2,000,000 aggregate.
- In addition to the Certificate of Insurance (COI), the submitted policy must include the following endorsement forms attached: Primary and Noncontributory, Waiver of Subrogation, and Additional Insured.
- The City of Rohnert Park must be listed as the Additional Insured exactly as follows: The City of Rohnert Park Its officers, agents, employees, and volunteers 5401 Snyder Ln, Rohnert Park, CA 94928.
- A copy of the SAM (Sexual Abuse and Molestation) policy form. If SAM coverage is a on a separate policy, a copy of the policy declarations page is acceptable.

Sessions
We offer three contracted program sessions each year—Winter/Spring, Summer, and Fall—providing instructors with consistent opportunities to offer classes throughout the year. This seasonal structure allows you to plan ahead, build enrollment, and grow your program while staying aligned with the City’s Recreation Guide publication schedule.
- Winter/Spring Session (January-April)
- Summer Session (May-August)
- Fall Session (September-December)
Class Proposals
Once the application and onboarding process are complete, you will be invited to submit a Contract Instructor Recreation Class Proposal, which allows instructors to propose classes for upcoming program sessions as part of the triannual contracting process.
