      1. Persons intending to start a new on-sale or off-sale licensed alcoholic beverage sales business, or..

      2. Persons who choose to substantially modify* an existing alcoholic beverage sales business.

      Once form is completed, print it out and obtain signature from the owner and submit to RPDPS Fire Division.

      *Substantially modifying your business includes:

      • A change to the type of retail liquor license with the Department of Alcoholic Beverage Control
      • An off-sale alcoholic beverage business who increases the floor area or shelf space devoted to alcohol sales by 25 percent or more
      • An on-sale alcoholic beverage business who increases the floor area devoted to alcohol sales by more than 250 feet
      • A business who proposes to reinstate alcohol sales after the ABC license has been revoked or suspended for a period greater than 30 days by the ABC
      • A business who proposes to reinstate alcohol sales after a cessation of use for a period of six months or more.
